• Meets with and interviews with clients, families and other care providers to assess client needs and eligibility of services, prepares mental status and social assessments and other client information summaries.• Develops treatment and service plans for clients. Makes and documents periodic reviews.• Participates in client review meetings and provides input into other case mangers' cases.• Responds to requests for information about available services for mentally ill, intellectual developmental disabilities and/or substance abusing child, adolescent and/or adult client populations.• Advocates needs of clients within and outside system, liaise between client, care givers and service providers.• Assists clients in obtaining basic resources such as: shelter, food, medical services, and disability benefits. Assists with client relocations.• Monitors client behaviors and progress and provides assistance to clients toward attaining pre-determined goals, schedules clients appointments, visits and observes residences and supervises medications compliance.• Provides supportive counseling to clients and client families, intervenes and provides emergency crisis counseling.• Makes client referrals to appropriate service providers and coordinates service delivery to client.• Participates in discharge evaluations and planning.• Consults with other agency professionals.• Attends and testifies at court hearings and other legal proceedings involving the client managed if applicable.• Coordinates services for persons who are HIV positive and have mental health, intellectual developmental disability and substance use disorder treatment needs.• Does related work as required.• Transports clients as necessary.

• Provide Person Centered Treatment, CBT for adults aging from 18 and up• Meeting with clients to evaluate their health and substance problem• Identifying issues and create goals and treatment plans• Collect UDS for assessment purposes• Teaching clients coping mechanisms such as mindfulness meditation• Helping clients find jobs or reestablish their career• Writing DAP Notes for documentation purposes• Leading group therapy sessions• Providing updates and progress reports to courts, social workers and probation officers• Referring clients to support groups• Setting up aftercare plans

• Provide documentation for districts including Monthlies, FAPT Reports, Ed-Cat Reports, Mid-years, and Yearlies.• Provides individual and group counseling to meet the developmental, preventive, and remedial needs of students• Consults with students, parents, teachers, and other school and community personnel to assist in meeting the needs of students• Assists teachers with the guidance curriculum• Provide daily support to students ages 5-18 by assisting them in modifying behaviors and assisting in the developmental of useful interpersonal skills• Provide direct assistance to students with hearing impaired and other students in the school setting, implementing lessons and reinforcing skills using PAWS-cards, and teaching them appropriate social skills• Assist with supervising SOL testing• Create, update, and utilize PBIS plans for students• Update Points and Levels data and graphs each block of school and week pertaining to the student’s behavior
