I leveraged technology to design a 10-day tracker for patients after their discharge, texting daily questions to assess their behavior and avoid readmissions.Building collaborative relationships with partner facilities to develop standardized processes and workflows, successfully decreasing the average skilled nursing facility (SNF) length of stay for accountable care organization (ACO) payers from 30 days per patient to 24, saving an average of $400 per day. Optimizing patient health outcomes, decreasing the utilization of post-acute facilities, and saving money by collaborating with hospital providers to create standardized reviews using InterQual criteria that determined an appropriate level of care placement.
