Executed HIT readiness assessments, workflows, project plans and project timelines. Worked with practices on clinical reporting. Familiar with many EHR systems reporting features and methods for querying data. Able to assemble data into reports and file these reports with CMS. Performed root cause analyses on practices to determine effectiveness of data, reporting and reporting outcomes. Developed strategies for behavioral changes to improve data entry, reconcile referred immunizations and cancer screenings and assist in the cleaning and maintaining of patient data bases.Recruit providers and FQHC's for the REC. Perform gap analyses for practices to identify current alignment with MU core and menu set items and prioritize work to comply with non aligned objectives. We are assembling beneficiaries, providers and stakeholders into Wiki's designed to enable beneficiaries to tell their medical stories, for providers to suggest strategies to improve chronic illnesses and for communities of health to identify best practices and share these ideas to reduce healthcare costs and improve the quality of care in Hawaii.
