Working is the Children with Disability team covering north Buckinghamshire. Clients will be young people from 0 to 18 years of age with a variety of disabilities including physical, learning and sensory. All aspects of social care, child protection, fostering, adoption, Looked After Children, children in care and leaving care, Family Group Conferences. Skilled in completing Initial Assessments, Core Assessments, viability Assessments, and Risk Assessments. Specialist area of interest and expertise is Transitions from Children's to Adult services.Specialist link worker for Transitions, working with the Transitions Commissioning Manager to ensure the Bucks CC Transitions Process is followed by both Children's and Adult services, supporting and advising both areas in all aspects of the process and procedures and acting as a specialist advisor to professionals and families to ensure all young people move through the transitions from children's to adult services smoothly and appropriately. I also give presentations to a variety of groups including professionals and families.
