Shared Living Provider
CurrentA Shared Living Provider (SLP) is someone who cares for and provides a home for a client withdevelopmental disabilities to help them with independent living skills, provide them with support, and create a safe and structured home environment. Shared Living is a flexible model of support that can meet a variety of people’s needs while honoring individual choice, relationships and community.Responsibilities:• Provide housing and day-to-day assistance, tailored to the different needs of an individual with developmental disabilities• Work with client to become more self-sufficient.• Assure care and supervision of the client twenty-four hours per day and that medical services are provided and documented.• Maintain regular and effective communication with the service coordinator for the client who lives withthem, and work cooperatively and facilitate relationships with the individual’s guardian and/or natural family.• Support and facilitate contact with the client's natural family.• Provide long-term stability and consistency in the client's life.• Maintain a safe and accessible household.• Hire, manage, and complete timesheets for a support team for the client.• Exercise good judgment and be comfortable working independently, while also understanding who to call for support in the event of an emergency.
