I am Podiatrist who is board certified by the American Board of Foot and Ankle Surgery and by the American Professional Wound Care Association. My experience includes 40+ years providing foot healthservices in nearly 200,000 patient visits, 20 years in private practice, and 20 years in the Veterans Affairs Medical System as former Chief of Podiatry Section and Chair of the Preservation Amputation CareTreatment (PACT) Program.My passion for excellence in prevention of the comorbidities of diabetes led me to develop the ePrevenir tool. It is a cloud-served, machine learning driven, clinical decision support system for value-based primary care providers for managing the four co-morbidities of patients with diabetes as well as patients categorized as pre-diabetic. The tool is to be used in the primary care provider office by ancillary staff to evaluate and manage the comorbidities of pre-diabetes and diabetes by fourmodules: the feet, eyes, kidneys, and cardiovascular system.Patient data feeds the Machine learning and Artificial Intelligence that provides information on the status of the patient’s current diabetes related comorbid conditions as well as a window to the patient’s futurehealth via predictive analytics and fusion of the data of all the comorbidities. This physician-focused software is the first of its kind in the industry.The ePrevenir tool promotes patient throughput and provides an evidenced-based medicine standardized method for evaluating each of the comorbidities. The first module focuses on foot health and identifies each patient’s unique risk factors for developing a foot ulcer. The tool offers recommendations to the Primary Care Provider for the management of the patient’s foot health as recommended by The International Working Group on the Diabetic Foot and Centers for Medicare. Additionally, the tool offers patient education and recommendations for self-care.
