Directly responsible to deliver on the vision, strategies and performance of cross-continuum care management (inpatient, outpatient and ED) and its contribution to value-based contract performance along with attention to hospital value-based purchasing, quality, patient safety, customer service and compliance needs. Accountable for integration of all care management strategies at the system level to ensure operations and work processes are efficient, innovative and support a shared structure of care coordination across the continuum.
