HHCC is an inpatient Hospice acute care facility. The eleven bed unit cares for hospice patients who have symptoms ( ie. pain, terminal agitation) that cannot be controlled in a home care setting. As a RN I work directly with our Interdisciplinary Group made up of Doctors, Nursing, Social work , Pastoral Care,and Bereavement Services

Care coordination for 2 bed comfort care home, oversee staffing, resident care, symptom management, work with hospice organizations . Also manage donations, funding and community relations.

Assessing patient overall health intelligence and working with them at their level with motivational interview techniques. Ordering durable medical equipment for patients and assisting patients obtain needed medication. Also, worked phone triage for the internal medicine department. Assessing patient medical issues and determining the direction of care, i.e. office visit or send to ED. Often helping patients avoid ED visits for minor issues that could be handled in the office, or avoiding delay of care for emergent situations.
