Facilitates the development, implementation, and monitoring of clinical programs under the Transforming Clinical Practice Initiative. Contributes to the identification, development and implementation of clinical interventions that further the aim of improved population health and better patient care provided at managed costs. -Participates as a key member in the clinical and operational strategies to support development, introduction, implementation, and evaluation of clinical programs at the small and large private practice levels and hospital levels.-Assists with monitoring quality improvement and efficiency work plan including identifying opportunities to meet and exceed performance targets. -Oversees the development and deployment of workflows, educational curricula and evaluation methodologies that support physician practice and hospital quality initiatives. -Assesses barriers when physicians and staff have not met reporting goals and are not following work flows .-Effectively coordinate the assessment of new clinical design projects for implementation at facility or facilities as assigned. -Organizes relevant task forces, reviews evidenced based literature/benchmarks, establishes indicators for monitoring and evaluation of quality and appropriateness of care, continuous improvement, and achieving targeted goals. -Collaborates on the design of the information technology infrastructure required to support a system wide physician quality and practice quality improvement system .-Participates in development of training/ and delivery of educational and evaluative materials and programs that incorporate PTN clinical practice. -Researches and disseminates best practices.

Job Overview:The Patient-Centered Medical Home (PCMH) Population Care Coordinator (PCC) works with a multidisciplinary health care team in a primary care setting. The PCC focuses on coaching and coordination of care for Horizon Blue Cross Blue Shield of New Jersey participating patients. The PCC works alongside the physicians and office staff in a PCMH, focusing on identifying the needs of Horizon BCBSNJ's complex members while assisting the practice to develop a process for managing Horizon BCBSNJ's member population. This person promotes patient-centered care, works side-by-side with primary care providers and medical home team members and actively participate in multidisciplinary patient-centered team meetings.Job Responsibilities:Develop constructive relationships with local hospitals admission offices, Case Managers, discharge planners and Emergency Room (ER) contacts.Work to develop systems, processes and initiatives to engage these entities in relevant case management activities with each Horizon BCBSNJ insured member of the PCMH to ensure that all necessary post discharge needs are met.Work with physicians, office staff and Horizon BCBSNJ Clinical Reporting (Informatics) to help identify high-risk, high-need and potentially high-cost patients
