1. Administrative intake of members or managing the admission/discharge information post-notification2. Working with (USA) hospitals and the clinical team, including managing incoming calls, managing requests for services from providers/members, providing information on available network services3. Transferring members as appropriate to clinical staff4. Manage the referrals process, processing incoming and outgoing referrals, and prior authorizations, including intake, notification and census roles*This includes making general and specific request of clinical information from dialysis facilities and providers. Faxes are received and processed by entering specific clinical data required into the system. This data is then forwarded to the case manager for their reference.

1. Participates in the implementation of various projects/programs/systems and activities related to Maternal, Neonatal and Child Health and Nutrition (MNCHN) And Pantawid Pamilya Program such as:a. Maternal Health Services1. Pre-natal Care2. Post-natal Care3. Family Planningb. Child Health Services1. Expanded Program on Immunization2. Nutrition3. Dental Health Programs2. Conducts regular monitoring of various health programs of the city.3. Verify/ Validate the compliance to the conditionalities of the Pantawid Pamilya recipients I. Monitor compliance of the households on the following: a. pre/post/ante natal care b. facility based delivery c. complete immunization of children d. school performance of children e. attendance to the learning and development activities for parents/guardians II. Ensure timely submission of Compliance Verification System (CVS) Form by the Rural Health Midwives.4. Conduct health education and training a. mother's class b. Nutrition Education c. Family Development Session5. Conduct disease surveillance6. Advocate on National Health Insurance Program (PhilHealth) availment and benefits7. Trainers and supervisors of Community Health Teams (CHTs)8. Data Management and Reporting (related CHT)9. Prepares and submits reports/project requirements to Department of Health-Center for Health Development Office a. DTR/ Monthly Journal Report/ Monthly Schedule of Activities/ Monthly Accomplishment Report b. Quarterly Assessment c. Application and Research Outputs > Case Study with Preventive Plan (Family and Community Level) > Project Proposal based on identified problem/issue
