Providing support and assistance to grant applicants.Transform Wisconsin Fund, administered by theWisconsin Clearinghouse for Prevention Resources andfunded by a Centers for Disease Control & Prevention Community Transformation Grant, intends to commit $6.6million over 26 months to community coalitions andpartnerships. The goal is to promote healthy environments that help Wisconsinites make healthychoices and therefore prevent chronic diseases.
