Hospice Home RN Care case manager. coordinate an ancillary team consisting of doctors, LPNs, social workers, chaplains, and CNAs in relation to nursing care of patients. Manage nursing care of up to 20 patients in home hospice care providing end of life care to patients. This involves working with people with such diseases as heart failure, COPD, cancer, Alzheimer’s, and others. End of life care includes care planning, pain management, wound care, and symptom management.  Provide wound care to necrotic venous stasis wounds, stage IV pressure wounds, deep tissue wounds, ulcerated weeping edema and more. Manage care of ports and central lines. Deliver emotional support and training in both in-person and non-face-to-face settings.   Observe and perform nursing assessment and timely documentation with every visit and promptly report significant changes to the appropriate person. Administer oxygen and medications according to company policy. Provide support and health education to patients and caregivers. Educate families and caregivers in the recognition of problems that would require them to call for assistance and am available by direct phone line to provide such assistance as needed. Respond to on-call needs. Facilitate face-to-face and phone triage.   Handle admissions. Experienced in working with culturally diverse populations both inpatient care and in the workforce. As the only Spanish-speaking nurse, handle nearly all Spanish-speaking patients. Patient encounter paperwork and documentation is completed on the day of the visit.

Through teaching and presence, ensured high standards for curriculum throughout CompassionInternational’s field offices. Managed a development team of up to 15 professional writers. In addition, delivered monthly orientation education for new employees.
