As the Director and Principal Consultant at Fivebyfive Engineering in Perth, Australia, I lead a team in providing risk management and safety case development services. My role involved facilitating workshops and implementing strategies to enhance safety protocols for our clients. We were able to ensure compliance with safety regulations and standards, mitigating potential risks for our clients, and ensuring acceptance of facility safety cases.

Responsible for the maintenance and implementation of the Prelude Safety Case. To date I have had three revisions of the Safety Case accepted by the regulator (NOPSEMA).

I am a Senior Risk and Safety Engineer at Genesis Oil and Gas. As part of this role I project manage and develop Safety Cases, Facilitate Risk Assessment Workshops, undertake detailed risk modelling analyses (e.g. Dropped Object Assessments, consequence modelling etc.), and play a role in the development of younger engineers.Recent projects include:- Woodside: NWS Facilities Diving Activities Addenda. First time as Project Manager, delivered 4 Safety Case Addenda (North Rankin Complex, Okha, Goodwyn Alpha, and NWS Pipelines);- Chevron Thevenard Island Offshore Wells P&A Safety Case (Drilling Safety Case for the Ensco 107);- Chevron: Drilling dropped object study for Gorgon Stage 2 at Manifold 1;- Woodside: Okha FPSO Greater Export Line Subsea Isolation Valve Remediation Addendum;- Woodside: Pluto Alpha Walk to Work Addendum (motion compensated gangway - first time this technology was used in Australia);- Woodside: Angel Export Line Pigging Addenda for Angel and North Rankin Complex Safety Cases.Throughout these scopes I have facilitated ~20 workshops.

Responsible for the development of an SCE Criticality Assessment, an Isolation Valve Leak Study and the revision of the safety case itself, plus numerous stand-alone Formal Safety Assessment documents. Chevon – Technical scribe for Wheatstone Process Hazard Assessment focussing on applying HAZOP techniques to the Wheatstone Procedures (Normal / Emergency Operations, Start-up / Shutdown etc.). The process involved a systematic step-by-step process applying specific guidewords to nodes within each procedure. A node consisted of a pre-determined number of steps within the procedure that provided instruction to complete a specific task. Recommendations were captured where necessary to mitigate hazards generated through the activities described by the procedure. Cooper Energy – Preparation of the Sole HDD Installation Safety Case. This involved the development of the facility description, summarising the Safety Management System for Cooper, and completing a formal safety assessment on the risks identified for the construction of the HDD section of the Sole pipeline. Baker Hughes – Facilitation of workshops focussing on pressure testing and dewatering operations for INPEX facilities. The workshop focussed on both the Central Processing Facility and the Floating Production, Storage, and Offloading facility.

Goodwyn A Offshore Safety Case Resubmission - key involvement in the five yearly update of the Goodwyn A safety case. This resubmission involved the update of all Formal Safety Assessment documents as well as the safety case itself. Throughout the update of this safety case, the following were also required:Development of bow tie diagrams using BowTieXP;Development of Concordance Tables; andDevelopment of regular progress reports that were delivered to the client.Vessfire Modelling - Use of the software Vessfire to create a model that determines the time to release for storage vessels, when exposed to jet or pool fires.Business development - Responsible for the preparation of proposals for new work and attempts to attract new work to the company.

Auditing - Undertook risk procedure audits on external contractors to ensure conformation to GUFT standard.Action tracking - Owner of Risk Register and HAZOP Action Register and responsible for action close out and progress tracking.Project Safety Cases - Key member in the development of the construction safety cases for the Gorgon, Jansz and Domestic Gas Pipelines. This involved the development of bow tie diagrams, the identification of major accident events and any associated performance standards.Risk assessment facilitation - Responsible for the preparation, facilitation and reporting for numerous risk internal and external risk assessments, including an AS2885 Pipeline Risk Assessment.Monthly reporting to upper management - Responsible for the development of a monthly risk report detailing the highest level risks on the project, and tracking the actions to mitigate these risks. This report had input from upper management and was shared with many high level stakeholders.Loss of Containment Study - Completed a loss of containment study identifying all the possible threats to the pipeline and the consequences of any loss of containment events.Participation in Company Incident and Injury Free workplace safety regime, including attendance at monthly Operational Excellence meetings. The project sponsored two training courses, including a four hour Incident and Injury Free workshop and a 2 day Behavioural Based Safety workshop. The project also encouraged the submission of any unsafe working conditions or behaviour observed in the office.

Saudi Kayan / DAELIM HAZOP – Scribed a one month HAZOP for an LDPE plant that was to be constructed in Saudi Arabia. This included the mark up of P&IDs, scribing the workshop and developing the workshop report. This workshop was held in Seoul, Korea.Apache Energy Limited – Ocean Legend Decommissioning Safety Case. Preparation of documents, scribing and report development for a number of workshops that were conducted to develop the of the Legendre Decommissioning Safety Case.HAZID Scribing - Responsible for the preparation, scribing and report development for various risk assessments.Subsea Isolation Valve Assessment - Key involvement in the development of a Subsea Isolation Valve Requirement Study, which determined the benefits of having an isolation valve offshore from an onshore processing facility.As Built P&ID Verification - Involved in an As-Built study for Kwinana Nickel Refinery, as their P&IDs were significantly out of date, as a result of a lack of management of change practices over many years.
