 Conducted individual, couples, family and group therapy for chronic and acute persons witha variety of psychiatric diagnoses. Performed crisis intervention, intake assessments, treatment planning, interventionevaluation, referrals, termination procedures and follow-up care. Periodically evaluated client progress and reassessed services needed. Maintained required records and monitored client’s mental and physical stability. Acted as a liaison for consumers with their primary care physicians. Collaborated with client’s family members, neighbours, friends, employers, teachers andother service providers in treatment team meetings and in the community to stabilize andreinforce social support for clients. Initiated community-based projects for other organizations, schools and hospitals.
