This role required me to work on an individual basis with clients aged between 40 and 74 with long term health conditions. Many of these clients also suffered with enduring mental health conditions and physical/learning disabilities. Based in the West End of Newcastle, I worked with a culturally diverse caseload, which often involved working with non-English speakers, requiring me to successfully utilise interpreting services. I worked with a large caseload of 100+ to help these people to manage their health better, through advising and signposting into relevant services.Using a semi-structured assessment and interview tool, I gauged the needs of the client and, together with the client, put together an action plan to help them to overcome barriers to managing their health. This involved issues surrounding finances, housing, mental health etc. This role required me to develop a trusting relationship with my clients using active, empathetic listening and motivational interviewing techniques. I also developed professional relationships through services in Newcastle who we regularly linked into, as well as the doctors surgeries who we received referrals from, in order to facilitate the referral pathway. Through this role, I developed exceptional case management skills, and regularly had to prioritise commitments and conflicting demands on my time. I regularly wrote clear and concise case notes and logged all client contact on our date data management system, which required me to put aside time to complete this, as these notes needed to be kept up to date to enable our team to recognise if we were hitting targets.I have also organised and facilitated educational groups, such as cookery and healthy eating sessions, and delivered regular educational talks to a local Pulmonary Rehabilitation group around the topics of anxiety and depression with chronic health conditions.
