Provision of clinical case management services to individuals on LPS Conservatorship/WIC holds on acute psychiatric admissions unit; participation in Probable Cause Hearings for 5150, 5250 and Temporary Conservatorships; facilitated psychosocial rehabilitation (PSR) groups & unit milieu therapy; court report recording for various county agencies; provision of clinical social work expertise as a member of multi disciplinary team; completion of all clinical social work, treatment planning and group documentation; clinical case management; risk assessment and evaluation; discharge and aftercare planning; brief crisis intervention; assisted in the implementation, development and coordination of PSR “Treatment Mall” curriculum at MSH during inception.

Completion of mental health and substance abuse assessments for General Relief/Cal Works participants referred by the Department of Social Services using standardized assessment tool; case management; brief crisis intervention. Completion of comprehensive psychosocial assessment documentation.

Per Title 22 regulations for three 6-bed group homes ICF-DD-H (Intermediate Care Facilities Developmental Disabled-Habilitative) for DD/MR adolescents/adults: Case management, coordination, monitoring, & ongoing program & objective evaluation of all 18 residents’ Individual Service Plans (ISP). Regular completion of on-the- job and in-service training with all direct care and supervisory staff. Policy and procedure evaluation, implementation and evaluation. Administrative oversight and facilitation of DHS licensing survey process and protocol.
