I worked as part of a team to provide a safe, effective, efficient care environment for patients, relatives and carers. I was responsible for the assessment, planning, implementation and evaluation of programmes of care without direct supervision.

I worked as a member of the General Practice Team and received patient referrals from the Team and other Health Care Professionals aligned to the Practice. I undertook clinical consultations and delivered care adhering to Patient Specific Directions and Patient Group Directions.

I was a Primary/Named Nurse within a multi disciplinary team, developing collaborative, needs based care plans, through assessment, planning, implementation and evaluation. I provided excellent professional, skilled and effective person centred, evidence based nursing care.

I provided care to patients and residents based on their care plan outlined by their doctor. I accurately monitored and recorded observations on patients' conditions. I maintained accurate records of residents care, financial procedures, and medication administration. I identified care planning needs.

I prepared patients for examination and treatment; distributed and served food, assisted patients in feeding and prepared snacks and hot drinks; assisted patients in washing, dressing, toiletry activities and general mobility; changed bed linen, made beds and tidied wards.

My main role and responsibility was to ensure individuals were receiving the full amount of care which was requested from them and for the standard to remain high with dignity and respect being kept at all times.

The primary role was to assist elderly or disabled clients with practical tasks that they couldn’t manage themselves or required encouragement to complete independently.
