Recover Health – Outreach Independent Living Skills Instructor March 2011- PresentAs an Independent Living Skills instructor, my main task is to identify my client’s needs in order to create individualized goals, long-term projects, or both with the aim of helping them obtain greater independence with minimal state assistance. To accomplish this task, I meet with them initially to assess their current situation and evaluate their level of independence and work with them to set goals aimed at increasing their level of independence. Each client/project is unique and needs to be dealt with in different ways to maximize the effectiveness of the plan set in place. Currently, I have 20+ clients/projects that I handle independently with weekly correspondence with my supervisor. One of the most important aspects of my job is finding suitable housing for clients in need. Each of these cases is unique when you consider age, income, level of independence and care needed for an individual to succeed. Certain housing facilities only accept elderly, mental illness, traumatic brain injury, or other individualized diagnoses. Other housing facilities are based solely on income and not the level of care needed. Diagnosing the level of care involves meeting with all team members (doctors, psychologists, physical therapists, case managers, family members) and detailing what must be met to enable a client to become successful. After this initial assessment is completed, I sit down with the client and brainstorm ideas of how to achieve these goals. Once a goal such as housing is met, I meet with the client and their team to focus on other goals that promote independence.
