Since July 2019 I commenced working within the Community of both Southwark and Lambeth Providing specialist palliative care. I am part of a small team of Senior Nurses who triage and then visit patients overnight within their homes. These patients are all at various stages of illness but all with a life limiting condition . Within these patients home I assess patients and formulate ways to best treat their symptoms in their home.If unable to resolve an issue within the home I will escalate to on call Doctors or the Palliative Doctors in the morning; handing over my assessment of the patients and recommendations . I work closely with the District Nursing team , taking referrals overnight from the Guys and St Thomas's switchboard, the 111 service and London Ambulance Service. A large part of the role is communication and support of my patients , their families and friends during an extremely emotional and distressing time .

Administering intro-ocular medication Assessing patients before discharge and educating them on post operative care once home including educating on the administration of various eye drops .

During my time at London Bridge Hospital I worked within the Day Surgery/Endoscopy department caring for patients both pre and post operatively; assessing patients. A crucial part of the role being assessment of the patients current health, needs and condition and acting on this in the relevant manner.

Within this role I would care for patients pre and post-operatively by Admitting and assessing care needs. Administering Medication Caring for post operative wounds, assessing these and notifying the medical team in case of emergency . Educating patients on how best to care for themselves when home and referring them to the appropriate resources such as Physio Therapists and their own General Practitioners. Supporting patients emotionally following negative news post examination .

Teaching challenging groups of children who were unable to assimilate into a classroom environment. Providing guidance and support for young people. Assisting in the acquisition of a new language for children who had come from abroad speaking no English .
