About Matthew Skidmore

Graduated from UWE Bristol in 2020 with a First-Class honours in Biomedical Science. During this time I focused largely on neuroscience, completing my Dissertation on the application of viral vectors for transport of therapeutics across the blood brain barrier. Specifically, novel, neuron-stimulating genetic therapy in Parkinson’s Disease. I was awarded a scholarship to study Physician Associate Studies MSc, graduating with a Distinction in 2022. Gastroenterology is huge and fascinating field. I was interested by the diversity of GI disease, as well as the interplay with other organ systems, even the neurological system via the “gut-brain axis”. It also offers many potential clinical skills and procedures for the junior clinician. Having started my first role is in the Wye Valley NHS Trust, this has provided an excellent opportunity to develop my knowledge in gastroenterology, particularly in the sub-specialism of hepatology. Having completed my first year at the, I am now fully competent in the day to day running of our inpatient ward, and have gone on add value to my role by developing a number of practical skills including, US-guided ascitic paracentesis, a skill which is crucial to working within this speciality, given a rise nationally in the prevalence of liver disease. Now there is consistently a ward-based clinician competent in these procedures, meaning rotating junior doctors have better access to training for these gastro-specific skills. I have completed a number of quality improvement projects within the department. One aimed at how to achieve timely and appropriate admissions to our gastroenterology ward from the front door, inspired by a poster presentation at the annual BSG conference. Through proposed interventions trialled in this project, the proportion of patients admitted under the care of our team, of whom had a primary gastroenterological working diagnosis/presenting complaint, increased from 25% to 75% of admissions. 6-months into my role I have started outpatient clinic reviews. Initially these are focused on achieving the gold-standard of a 6-monthly review of our cirrhotic liver patients. Additionally, I see recent discharges within 1-2 weeks in a “hot clinic”. This had not been done routinely before in the department. This approach aims to reduce the rate of readmission in vulnerable patients, addresses issues encountered post-discharge and ensures appropriate follow-up is arranged.
