As part of the NYS Medicaid 1115 waiver, CBOs will now be paid for performance to address social determinants of health for their communities. Together these CBOs will form Social Care Networks under the guidance of regional agencies who will act as SCN lead organizations. SCN leads monitor, assist, and pay them accordingly. In my role, I am responsible for the development and strategic execution of a performance measurement system to capture the work of CBOs across Long Island, NY. I oversee the provision of technical assistance, training, and reporting to ensure that Long Islanders are receiving the care and support they need. Through SCNs, communities have the opportunity to achieve more equitable and just health outcomes for all.

• Recruited by the VP of Infectious Diseases to launch 2 new grants and revitalize a third, bridging three teams into one unified collaborative effort to standardize HIV Prevention services across the organization • Responsible for the development and oversight of Pre-Exposure Prophylaxis and Post-Exposure Prophylaxis services and related sexual and behavioral health service provision across 11 clinics. Priority populations include LGBTQ-GNB PoC, YMSM. • Served over 3,000 clients with only 2 known seroconversion events. Managed a team of 8 staff to deliver services which include linkage and referrals, mental health and substance use screening and brief intervention, sexual health education, treatment adherence support, risk reduction counseling, motivational interventions, etc.• Additional responsibilities include the development and oversight of two borough-wide community engagement campaigns designed to educate people about HIV prevention and reduce stigma surrounding services. Both digital and face-to-face efforts.

Health Systems Change Coordinator, consulting with medical and behavioral health care organizations to implement evidence-based health screening and treatment interventions. Interventions are based on the Chronic Care Model and involve policy development, electronic health record utilization improvements, billing assistance, training, and culture change.Launched and facilitate an advisory board (Behavioral Health Task Force) dedicated to policy and resource development for NYC-behvaioral health clinics.

Collaborated with the Juvenile Justice Coalition to revise policy on sexual health education and services concerning youth in state care, including the NYC Administration of Children's Services, and NYC Department of Probation;Developed a legal/ethical toolkit for social workers interested in working with people affected by HIV;Conducted outreach to state-level stakeholders to form grassroots campaigns to advocate for decriminalization of HIV across the US.
