• Support consumers who live with mental health disorders and substance use to navigate through the systems to ensure their public safety and overall lives through community-centered approaches & interventions.
• Refer clients to resources for proper housing, employment-related services, counseling, and other means of reentry into the community.
• Collaborate with treatment teams to develop PCPs and assessment tools to evaluate risks and offer quality service to consumers. Maintain a person-centered approach—the ability to implement trauma-informed principles.
• Participate in clinical and administrative supervision, morning staff meetings, in-service training, and other staff development activities. Integrate peer voices in crisis planning, policy, and evaluation conversation.
• Utilize ACT Services and Crisis Planning to provide training and assistance in ordinary community experiences and integrate specialized mental health programs in or near natural community services.
