As Hospice Liaison, I evaluate a patients need/qualifications for hospice services as per medicare guidelines. I meet with potential pateints and families, explain our services, answer questions, and provide support, contact patients physician, and obtain orders for care. I also coordinate with the Moorestown Hospice team with the initial plan of care. I develop and maintain relationships with hospital and facility staff, i.e. physicians, residents,nurses, case managers, social workers, and hospital staff. I troubleshoot issues as they arise. I also develop and perform educational inservices for our referral sources and their staff members. I have met and exceeded my goals consistently each month.
