Increasing the Federally Qualified Health Center's capacity to serve low-income and uninsured/underinsured patients across the Hudson Valley through community-based primary care.1: Outreach patients with gaps in care with a focus on uncontrolled diabetes and hypertension, providing health education and scheduling appointments.2: Coordinate the Food is Medicine program in partnership with Feeding Westchester for a 120-patient cohort by recruiting patients with diabetes and food insecurity, ensuring retention through outreach, tracking participation, and executing four food distributions per month.3: Capture retinal images for diabetic retinopathy screenings with EyePACS and follow-up on referrals to ophthalmologists.4: Conduct intake screenings and follow-ups for patients receiving medication-assisted treatment (MAT) for substance use disorders.
