Managed care for 45 - 65 adult patients with mental health diagnoses.Services include but are not limited to strength and need-based therapeutic case management, connection to benefits support and resource development, mediation services, evidence-based therapeutic interventions, advocacy, crisis intervention, and family support.

Managed a caseload of 25 kin-care patients.Connected patients with social and medical needs to services and programs, including external funding and city resources during the pandemic lockdown.Services provided also included individual and family counseling and advocacy.
