Ongoing contracted Adult Social Services agency work. Have completed employment in Harpurhey Integrated Neighbourhood Team. Currently working within the Central Learning Disability Team as a targeted reviewer.

Social workers partner with people to address personal difficulties and structural barriers in their lives. They work with individuals, families, groups and communities.In this acute role, I directly oversee all the wards which has meant a lot of experience with different ages, illnesses and trauma. I am required to work off incoming referrals which I assess based on a priority assessment tool. I manage a high and demanding caseload daily. In this role, I routinely work with children, adults, families and communities across a wide range of units and specialty areas such as maternity, paediatric, medical wards, oncology, emergency departments, trauma, palliative care and more

Wagga Wagga Rehabilitation Services provides rehabilitation across the Murrumbidgee Local Health District and aims to support you to maintain independence during and after injury or illness.Within this role, I work across both the inpatient and outpatient (Ambulatory Rehabilitation Service) and manage 16 beds. I regularly complete psychosocial assessments, NDIS applications and liaise with MyAgedCare and various other support agencies to ensure patients are discharged with appropriate support services. This support includes; assessment, crisis intervention, grief and loss counselling, adjustment counselling, general counselling, patient education, advocacy and mediation, practical assistance (accommodation assistance, Centrelink & financial assistance), assistance with discharge planning and referral to other agencies.

Facilitated best health outcomes for clients requiring pre- or post-hospital care. Conducted comprehensive client assessments, arranging for service provision to clients and exiting clients from the program upon service completion. Skilled in developing care and service plans, maintaining progress notes while monitoring and adjusting services as needed. Ability to work with clients with diverse cultural, emotional and social needs with health goals ranging from rehabilitation, to disability support, to end of life.

Family Based Care, Central West. January 2021 – April 2021Direct placement within a Case Management role. Effectively communicated with various different age groups to build rapport and relationships. This placement also gave me the ability to problem-solve under difficult scenarios, and gave me the foundations to multitask and triage my workload.

A mixed-mode placement during which I developed a Telehealth service available to various outlying hospitals throughout the Murrumbidgee Health District. Completed and observed various psychosocial assessments. Demonstrated ability to target interventions to reduce the negative impacts of illness and hospitalisation.

This role included personal care, domestic assistance, meal preparation, social support, transport and professional documentation. I had the opportunity of leadership through training various new staff members, a responsibility I found offered a lot of professional development. Within this role I was also able to showcase my high level of enthusiasm and further develop my communication skills and ability to work with a variety of individuals.
