• Provide and coordinate a wide range of supportive and therapeutic mental health services for a caseload of up to 100 adults with severe mental illnesses and addiction issues, with specialization in supporting transgender and gender expansive individuals• Assess and complete referral letters for transgender and gender expansive individuals seeking gender-affirming procedures• Complete mental health assessments and level of care determinations and diagnose using DSM V criteria• Develop, implement, and evaluate the effectiveness of service plans tailored to each individual’s needs• Provide crisis assessment and intervention services to assist clients in minimizing or eliminating periods of crisis, severe dysfunction, and acute care utilization• Facilitate trauma-informed groups and individual therapy utilizing illness management and recovery concepts as well as multiple therapeutic modalities • Engage in advocacy and case management services to help clients secure benefits, access housing, medical, and emergency services and assist clients in developing and accessing natural community supports• Provide on-call support at Cascadia's Urgent Walk In Clinic on weekends/evenings.
