CurrentSt. Ann’s hospital is one of four full-service hospitals in the Mount Carmel Health System, which has 10,000 employees, 2,000 physicians, and 900 volunteers in the central Ohio region. St. Ann’s offers comprehensive services including a cardiovascular center of excellence with open-heart capabilities, a Primary Stroke Center, a Women’s Health Center, Maternity Pavilion, Network Cancer Center, and a dedicated orthopedics and spine unit.-Served as an Epic Super User during transition to Epic EHR system from Cerner in October 2021. Identified issues early on and throughout transition period to normalize new workflow quickly-Member of Inpatient Pharmacy Operations Rapid Decision Group, which addresses workflow and operational challenges to maximize efficiency and patient safety-Verify physician-entered medication orders for correct dosage, appropriate therapy, safety, and efficacy for patients in the ER, ICU, surgery, stroke, labor and delivery, and observation units throughout the 281 bed short-term acute care hospital. -Perform checks of compounded sterile medications prior to dispensing, ensuring aseptic technique and USP <797> compliance-Review patient medication histories and collaborate with physicians in order to ensure appropriate continuity of care with regards to medications during inpatient stay-Interpret laboratory results and adjust medication therapies when appropriate, including careful monitoring and adjustment of renally-dosed medications-Provide pharmacokinetic dosing and monitoring for patients receiving Vancomycin or aminoglycosides.-Attend codes and STEMI alerts as required to facilitate expedient medication delivery during emergency situations-Respond to stroke alerts as necessary in order to provide fibrinolytic therapy at bedside when indicated-Participate in multidisciplinary rounds as required
