▪ Mentor an advanced standing Master of Social Work student intern and provide scheduled supervision▪ Facilitate the development of direct clinical practice, coordinating services, formulating assessments and treatment plans, case management, advocacy, integration of theory, intentional use of self, application of intervention approaches, critical thinking and interdisciplinary collaboration

▪ Assist residents and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care▪ Collaborate with other healthcare professionals to evaluate and treat residents to promote the understanding of one’s social and emotional factors underlying their health needs▪ Facilitate care conferences to encourage resident and family participation in care planning and understanding medical recommendations▪ Provide referrals to community resources to maximize the resident’s quality of life▪ Prepare social histories, preliminary assessments, quarterly assessments, annualassessments and discharge plans for residents▪ Develop and implement care plans and behavior management plans▪ Participate in leadership committee▪ Lead psychotropic drug committee with interdisciplinary professionals to ensure thateach resident is on the lowest and most therapeutic psychotropic drug regime▪ Ensure informed consents for each psychotropic drug is signed by a resident’sresponsible party and provider▪ Provide supervision and meaningful shadowing experiences for social work students▪ Train social work designees

▪ Develop, maintain, evaluate the social service program▪ Provide supervision to the social work and admissions department staff▪ Implement policies for the identification of the medically related andemotional needs of each resident▪ Participate in the admission process and discharge planning▪ Direct a monthly psychotropics drug committee in conjunction witheach resident’s primary care physician, psychiatrist, pharmacist and nurseto monitor and review for potential medication reduction▪ Organize and track risk vs. benefit drug statements for the use of eachpsychotropic drug for every resident in the facility▪ Facilitate weekly groups based on coping skills, problem solving,anger and conflict management, health and wellness, self-control, boundaries▪ Coordinate care conferences with each resident, families and staff to reviewcare plans and behavior plans▪ Provide individual counseling and therapeutic crisis intervention services▪ Coordinate behavioral health activities with other departments
