• Identify and contact eligible community providers and practices in site catchment areas to create awareness of the Michigan Opioid Collaborative (MOC) Program while also assisting with program enrollment and identifying necessary clinic and/or patient population resources needed from the MOC program.• Develop collaborative relationships with community providers and participate in behavioral health and substance use disorder prevention, treatment, and participate in six Upper Peninsula recovery coalitions/groups. • Provide at minimum monthly public educational and informational meetings on substance use disorder (SUD), Medications for Addiction Treatment (MAT), and services available through the MOC to groups with up to 50 attendees.• Provide linkage between community providers and MOC SUD specialists for patient consultations and guarantee patients are being treated in a timely and appropriate manner.• Facilitate coordination of care for patients with Opioid use disorder (OUD) and/or SUD by providing resource information for patients based on recommendations made by the MOC MAT Specialist Physician.• Provide face to face behavioral health and case management support in primary care office for patients receiving Medications for Opioid Use Disorder (MOUD). • Perform approximately four bio-psycho-social intake assessments per month for new patients and determine Opioid Use Disorder Diagnosis. • Deliver brief interventions including psychoeducation, motivational interviewing, crisis care, and counseling and community referrals for housing, food, etc., to patient(s) to ensure the necessary care and support to manage their substance use disorder SUD/OUD.• Oversee and complete weekly data collection through Qualtrics system, as well as assist with project evaluation to ensure program objectives are being completed as needed while also identifying and troubleshooting any potential setbacks/issues with the Data team.

• Responsible for oversight and ongoing development and implementation of the YMCA’s National Reach & Rise® Mentoring Program• Recruit, train, and successfully match volunteer mentors with youth ages 6-17• Manage Risk • Ensure uniformity of training curriculum, data collection, outcome objectives, and ongoing clinical support• Conduct comprehensive assessments and implement mentee (youth) growth plans, utilizing various intervention methods and techniques• Work in collaboration with other community organizations to provide optimal coordination of care• Established community resource networks and contacts• Provide ongoing case management • Manage program finances in accordance with federal grant stipulations

Ensuring a safe environment for adults 18 years old and up in an adult residential treatment facility for treatment of Addiction and Mental Illness.Managing a case load between 4-6 clients and developing an Individual Treatment Plan for each client for recovery from substance abuse Meet weekly with each client for an Individual Therapy Session to help them identify and overcome addictive behaviors. Provided client centered counseling using motivational interviewing and the 12 step recovery models of care. I also Lead Women’s Dialectical Behavioral Therapy and Women’s Intensive Relapse Prevention processing groups weekly. I support clients in transitional planning including securing aftercare treatment, finding stable housing, and access to employment/educational services and other community resources.

• Perform Mental Health and Substance Abuse Level of Care Assessments in the Emergency Departments of both medical hospitals under Alexian Brothers Health Systems and the Access Department at the Behavioral Health Hospital• Complete Level of Care Assessment, Five Axis Diagnoses, and provide clinical recommendation to clients of all age groups, ethnicities and religions• Establish and maintain a therapeutic rapport with client and family• Collaborate with Emergency Room physicians and nurses to provide safe, adequate and clinically appropriate care for the client• Consult regularly with team of psychiatrists under Alexian Brothers Health Systems regarding clinically appropriate recommendation for each client• Complete Precertification with insurance companies for treatment services recommended to the client• Initiate and maintain professional relationships with other social service professionals in the community including hospitals, schools, and police departments• Responsible for thorough completion of clinical documentation on each client case• Facilitate transfer of patient to appropriate level of care

Provided Crisis Intervention Services to runaway and homeless youth and their families. Provided short term counseling to stabilize youth and family. The goal of The Bridge Crisis Team is to preserve or reunify families whenever possible.
