Provide intensive case management to individuals in the LEAD program. Providing Outreach, long term engagement and supportive services for participants through intensive case management activities and collaboration with program partners, service providers, housing providers and other community organizations. Plan, organize, develop, and direct the overall operation of the Social Services Department in the facility. Ensure that the medical and psycho social needs are met and maintained for each resident . Conduct intensive case management including, HMO authorizations and continued services, coordinate discharge planning and community referrals. Oversee behavioral management and Psychotropic medication reviews. Stay abreast of compliance and regulations and provide education to staff, families and clients.

Served as initial point of contact for referral sources seeking education and information for their staff and patients on Hospice and Home Health services.
