I am working on a 3 year grant project funded through the Centers for Medicaid and Medicare Services. This demonstration project is called Multi-Payer Advanced Primary Care Practice (MAPCP) Demonstration. The demonstration focuses on the advanced primary care practice (patient-centered medical home) model; a team approach to care with the patient at the center that emphasizes prevention and uses health information technology, care coordination and shared decision-making between patients and their providers to improve chronic illness and preventive care. I will be responsible for preparing contracts with the physician practices and insurance payers that participate in the project.
