As the Admissions Nurse, I was responsible for performing initial and follow up physical and medical record assessments, identifying life expectancy, assessing care needs, collaborating with the hospice medical director, community physicians, community partners, and coordinating patient/family care needs with the interdisciplinary team as well as providing preventative and supportive care to the terminally ill patient/family unit.
