  As an RPM nurse, I provided monthly monitoring of vital signs, patient education, support, and communication with providers to facilitate better patient outcomes. I also trained new RPM nurses, audited notes for effective improvement, and provided solutions to improve workflows and problems within this start-up organization. I also facilitated the CCM program for new practices contracted with Athelas.
