MAP SITE ADVISOR / NURSING DEPARTMENT LEADJan 2021 through March 2022Responsible for overall operation and supervision of the facilities Nursing Program.Supporting 23-28 youth in co-mingled Congregate Care / Residential-Education Facility-Site Advisor for Medication Administration Program -Communication with DPH regarding COVID Vaccinations and Medication Reporting-Scheduling and Follow-Up of all medical needs including complex care and psychiatric care management.- Communication and Coordination with Attorneys, Court Systems, State Agencies, Guardians, and providing reports to state agencies.- Providing Health Education - Leading Supervisor Meetings with Updates on all Nursing Topics- Participation during Audits, communication with Auditors and State Licensor- Intake and Discharge Assessments - Fielding Referrals-Monitoring all Medication and Nursing Records and following up on any discrepancies in accordance with Policies outlined by the Licensors.-Periodic Audits of Medication Programs on site- Ordering Refills, Tracing Refills, Tracking Authorizations, Tracking Medication Supplies- Coordination of Medical Care including sending referrals, obtaining specialty authorizations, appealing denials.ADMINISTRATIVEThis position has been approved by the state to ensure the safety and continued transparency of the site's medication program.)COMPLETE TURNAROUND OF SITES MEDICATION ADMINISTRATION PROGRAM-Within 1 year brought the program into compliance with the state and re-secured MAP Licensing for Site and was a major contributor in the rl-licensing by DESE, DEEC.-Sites MAP Inspection Pass Rate went from 0% (based on companies last 2 yr) to 100% (3 yr in a row - 9 inspections with 100% pass rate)- On-Call Support- Contract Management- MIIS Setup (bringing the facility into compliance with DPH)- NPI (National Provider Identification) Setup for Facility- Participation in Ongoing Investigations with State Agencies- Staff Certification Management
