Conducting Community Access for Disability Inclusion waiver assessments of the 18-64 demographic. This role is designed to determine supports and services that best suit the clients needs in an effort to assist them in acquiring, managing, and sustaining additional supports to ensure they can remain in the community, meeting their social, physical, emotional, and mental health needs, while at the same time promoting personal independence. This role also requires the ability of the assessor to work with collaterals to assist in meeting the needs of the clients, as well as working the Customized Living facilities. In addition, the role requires correspondence and collaboration with the Minnesota State Medical Review Team and the Social Security Administration, as well as other county agencies that may be the County of Financial Responsibility or County of Service. This role requires internal department collaboration with LTSS Scheduling department, Office Support, Economic Supports, and Contracted Case Management Supports.

Case manager of assigned child protection matters that involved physical, mental, and emotional abuse, educational, medical and mental health neglect, abandonment, substance and alcohol abuses. Provided referrals for appropriate social services identified as essential in meeting the needs of the case and the independent needs of family members. Provided court documentation describing progress of case, additional needs, corrected concerns, and recommendations for further services, as well as consultation with county supervision and county attorney’s office to discuss needs of permanency resolutions to include that of reunification, out of home placement, foster care, and/or termination of parental rights.
