Behavioral Health Case Manager
CurrentProvide targeted case management services to patients on psychiatric unit. Manage a caseload involving complex case work in difficult mental health areas. Monitor complex, unstable, or chronic cases to successfully carry out treatment plans for chronically mentally ill clients. Assist in the development, implementation and monitoring of long and short-term treatment plans with the aim of improving and/or restoring individual, family and /or restoring individual, family, and community functioning. Recommend, arrange, and coordinate client residential placements. Monitor client stability in various aspects of community functioning.Assist in crisis intervention services over the phone or in the field.Assist in determining the triage level for referral to appropriate services. Maintain continuous case management services of a complex caseload over extended periods of time and for various levels of care. Assist in the coordination of treatment including the development of specialized resources, referral/linkage/transportation to treatment providers and specialized services. Assist in coordination of medical follow up for clients with concurrent medical conditions under the supervision of clinical, medical, or nursing staff. Assist in the assessment of clients in transition to a lower level of care. Prepare and maintain concise case records, case narratives, reports and documentation. Provide detailed reports for clients under conservatorships. Document client progress to maintain a permanent record of client stability, de-compensation and/or progress.
