Oversight for LIFE (PACE) center operations and the function of the interdisciplinary team (IDT) to assure optimum team performance, participant satisfaction and operational efficiency. Development and implementation of Inspira LIFE’s marketing plan to ensure continued program growth. Responsibility for enrollment staff, as well as coordination between enrollment and operations.•In conjunction with the Medical Director and Nursing Director, implemented several improvement initiatives to reduce hospital utilization. These initiatives reduced the organization’s hospitalization percentage to five percent in 2017 from ten percent in 2015, yielding a savings of $1.6 million dollars in hospital care.•Establishment of Inspira LIFE’s Behavioral wellness program, including psychotherapy and psychiatry, resulting in twenty psychiatric inpatient hospitalization days versus the median PACE benchmark of two hundred days in 2017. •Establishment of the first PACE Alternative Care Site in New Jersey by collaborating with an adult day care vendor, the New Jersey Department of Health and New Jersey Department of Aging Services thereby establishing a growth platform without capital investment.•Operational lead on selection and implementation of systems a new Electronic Medical Record leading to more comprehensive care coordination processes and timely care delivery.
