Assess needs of up to thirty clients residing within the community with a mental illness or medical diagnosis and develop a service plan for clients to meet their goals. Link and connect up to thirty individuals in the Broome County area to community resources needed to maintain independent living. Coordination of services between care management and treatment team to ensure overall wellness of consumers. Maintain case records in accordance to OMH and DOH standards. Also worked in Catholic Charities residential programs as an assistant supervisor and residential counselor where I would provide guidance to fourteen clients diagnosed with a severe mental illness.
