Since starting my private practice in 2017, my goal has been to create a supportive, client-centered environment that embraces diversity and meets the unique needs of each individual on their healing journey. As part of the Assertive Community Treatment (ACT) team, I worked collaboratively with other professionals to provide intensive, community-based support to individuals with severe mental disorders. With a caseload of 80 patients, shared in rotation with my team, our goal was to deliver comprehensive care that extended beyond traditional clinical settings.In my role as a vocational specialist, I focused on helping clients navigate barriers to employment and improve their functioning in real-world settings. I worked closely with clients to assess their vocational goals, develop job-readiness skills, and explore employment opportunities that aligned with their abilities and interests. Additionally, I provided ongoing support in areas like resume building, interview preparation, and workplace adaptation.In addition to vocational support, I was involved in case management, helping clients access necessary resources such as housing, healthcare, and social services. My work involved building strong relationships with clients to ensure they remained engaged in their treatment and care plans. I also collaborated closely with psychiatrists, nurses, and other mental health professionals on the ACT team to coordinate holistic care that addressed both the psychological and practical needs of each individual.The ACT model enabled me to provide consistent, personalized care for individuals with severe mental illness, ensuring they received the comprehensive support needed to live independently and manage their conditions in the community.
