Will lead and support efforts by utilizing a data-driven focus to prioritize initiatives to align Top 100 facility metrics and maturity pillars with the aim to align with Top 15 Hospital systems. The coordinator will ensure initiatives align with current evidence-based, best practice recommendations and will use clinical databases and national benchmarking to continuously identify opportunities for improved outcomes. - Accountable for facilitating progress toward desired outcomes related to mortality and readmissions- Utilizes process improvement methodology- Develops strategies for improvement that includes specific outcomes and metrics to monitor progress to a goal and make recommendations for improvement- Engages stakeholders across the system to ensure process alignment and prioritize resources; validates the merit and applicability of initiatives across functions related to Mortality and Readmissions- Contributes and co-creates with internal partners in the development of project management practices such as processes, tools, reporting and dashboard- Uses data, such as Premier Quality Advisor, Insights and PINC AI to analyze and drill down to identify opportunities to improve- Collaborates with the staff to identify systems issues that impact the care delivery system - Collaborates with Population Health to identify and optimize resources impacting patient mortality and readmissions- Remains current with trends in outcomes management and provides the staff with the resources and environment conducive to learning- Facilitates change and fosters a positive image of Clinical outcomes management Show less
