Resettlement Case Manager
CurrentThe primary purpose of the Resettlement Case Manager is to provide information, support,guidance, accompaniment, and service coordination to refugees and other newcomers inKingston and Canada, help clients to access needed health, educational, community,employment and specialized services, navigate systems, and establish themselves in thecommunity.Main Responsibilities:- Assisted clients in accessing essential health, educational, community, employment, and specialized services, adeptly navigating systems and facilitating their integration into the community.- Conducted culturally-sensitive needs assessments with a focus on gender intelligence, contributing to the development of personalized resettlement and integration plans.- Assisted participants in identifying short-term and long-term needs, goals, and priorities, and collaboratively developed associated action plans.- Facilitated service coordination by making referrals, offering accompaniment, providing advocacy, and offering interpretation support.- Delivered practical information on a diverse range of settlement-related topics in both individual and group settings, addressing questions and concerns effectively.- Provided solution-focused short-term counseling and crisis intervention, empowering participants to overcome challenges and enhance their well-being.- Equipped participants with the skills and strategies necessary to independently access government programs, community services, and education and healthcare services.- Collaborated with internal and external partners, as well as volunteers, to ensure participants' needs were met and necessary services were accessed, offering interpretation support as required.- Actively sought out and provided additional support as needed, demonstrating a proactive approach to fulfilling participant requirements.
