• Assure proper placement of patients within the home infusion/home health setting by gathering preadmission information, assist in better communication of information about the patient to all disciplines, and oversee the patient’s transition from acute care to home infusion setting• Educate patients, their families and facility staff regarding all applicable services and products and provide pre-discharge training on infusion administration in the home• Interact with area service providers on a daily basis to conduct on-site evaluation of patients selected by the hospital for home infusion services• Communicate timely with area discharge planner/social workers and home care providers to coordinate home infusion services for patients• Maintain positive relationships with referral sources and serve/act as a liaison between the hospital and home infusion/home care setting • Effectively communicate with agency staff, medical team, patients, and family in an effort to implement an effective treatment plan, explaining the available options, including advantages and disadvantages of various infusion delivery methods, ambulatory infusion pumps and vascular access devices• Review patients' medical records to obtain primary and secondary insurance payor information, and relay the information to the referral and intake department • Effectively communicate with employer's account managers/sales and participate in development and presentation of home infusion educational programs, in an effort to promote best practices and optimal clinical outcomes
