Throughout this clinical placement, I saw patients with varying health concerns including: - Weight management, heart failure, heart disease, Diabetes and GORD. - Tailored consultations to each individual client depending on priority nutrition issue- Formulated the nutrition diagnosis and tailored treatment plan- Delivered nutrition education session on Heart Failure in a community rehab program

In this Foodservice placement, my responsibilities included:- Engaging with stakeholders in the foodservice department- Identification of gaps in the room service model impacting patient satisfaction and nutritional intake.- Development of recommendations to bridge gaps identified- Regular liaising with foodservice and nursing staff - Development of data collection tools for audit

In this clinical placement, I was able to - Deliver nutrition education sessions to cardiac rehabilitation patients- Formulate a nutrition diagnosis and tailor a treatment plan to each individual - Observe and treat patients in the Diabetes clinic - Treat patient conditions ranging from Diabetes, heart failure, liver failure, heart disease, GORD, oncology and weight management

In this acute clinical placement, my knowledge of disease states was considerably built upon:- Collect, interpret and collate patient data to formulate a nutrition diagnosis and treatment plan in coordination with the treating team.- Communicating across multidisciplinary teams to ensure awareness of current services for each patient and that each patient has the appropriate care.- Liaising with the Dietetics team each morning regarding patient care.
