All work is underpinned closely with the Care Act, Mental Capacity Act and Human Rights Act, with anti-oppressive practice.Please note: My role has changed this year, due to Covid-19 and I am no longer based permanently in the hospital. Social Workers are now following the D2A (Discharge to Assess) protocol, and I see patients following discharge, in the community. Many cases involve safeguarding investigations, mental capacity assessments with best interest decisions, referrals to other services, and reviews of packages of care and placements, as well as Covid welfare checks. I now work from home, from the community office, or from the hospital (on occasion).
