I completed my clinical practicum placement at the Daniel Drake Center in long-term acute care (LTAC). As a practicing clinician, I had the opportunity to further my knowledge and provide appropriate services to improve my patients quality of life. Throughout this job, I got exposure and experience working with coma recovery, cognition, swallowing, receptive/expressive language, fluency, social pragmatics, and much more. The majority of my patients were weaning from the ventilator and on trach collar. I also had the opportunity to briefly work in the skilled nursing facility and with individuals receiving outpatient services. I conducted several evaluations including Modified Barium Swallow Studies (MBS), Fiberoptic Endoscopic Evaluations (FEES), Passy-Muir Valve (PMV), Rappaport (Coma Recovery Scale), Cognistat (Neurobehavioral Cognitive Status Examination), etc.

Observed Marianne Enginger, Speech Language Pathologist CCC, while traveling to different homes of adult and geriatric patients.Plans of care/goals for patients observed include: • Voice disorders; Oral, Pharyngeal, Esophageal Dysphagia • Cognitive Deficits; Augmentative Communication (Dynavox, Passy Muir Valve) • Dysarthria; Oral and Verbal Apraxia,; Expressive and Receptive Aphasia The following are activities performed for patient’s plans of care: • Word finding and confrontational naming, oral and verbal imitation • Oral motor, lingual and labial, and pharyngeal strengthening exercises • Calendar, orientation tasks, short term memory, immediate memory, and recall tasks • Breathing coordination exercises, vocal strengthening exercises • Practice tasks with Dynavox, SnapCore + • Education to caregivers and patients regarding their home exercise programs
