These referrals will need to be screened, verified, logged and forwarded to the appropriate team/department.DCU officer will be the main point of contact for the majority of discharges, where further non-acute services are required, they must have excellent understanding of discharge pathways from the Trust, so flow is not impacted upon. The post holder must be able to communicate and answer queries from all disciplines including care providers, members of the public and users of the service.The DCU officer will collate, interpret and analyse data to support patient flow, reducing length of stay within the acute wards. This involves close liaison with ward staff and community colleagues to ensure patients are discharged safely and within agreed timeframes.The role requires high levels of inputting accuracy, interpretation and understanding of data in relation to patient discharge.
