I am broadly interested in health policy, process, and outcomes, especially related to social determinants and disparities. I have spent much of my career honing skills related to quality management, peer learning, and health systems strengthening and have special expertise as these relate to HIV/AIDS patients and services. My unique combination of people skills, data analysis, and strategic thinking are compounded by my sunny disposition and enthusiasm to do great things together!The world’s most complicated health challenges can be resolved through meaningful community engagement. In particular, I am fascinated by collaborative learning and the power of health-related campaigns to raise awareness and diffuse innovations that improve outcomes, reduce disparities, and reduce financial and administrative burdens. By leveraging mixed methods data analysis, my team and I focus on true root causes to morbidity and mortality. Using think tank style focus groups involving the range of civil society stakeholders on a given issue, novel local public private partnerships create an all-hands response to the issue. After all, the most important thing in life is people and we can all accomplish more when working together!

Our toolkit includes six services that blend together to creat customized projects designed to End the HIV Epidemic, end racism in healthcare, promote local economic resilience, and to create a human resource pathway for the development of key community partners in service system planning and evaluation. Our toolkit includes:1. Storytelling Services: leveraging the power of community stories to build a qualitative information base of what is happening in the community2. Public Reported Evaluation Measure Services: working with community and other stakeholders to establish validated public reported measures using apps, call centers, and other methods3. Community Curriculum Services: establishing competency frameworks for training participants in evidence-based co-design activities from the perspective of the service population (not what the system seeks to extract from them)4. Community Assessment Services: working with a range of stakeholders to creat novel, incentivized, community assessments that include a mixture of survey, focus group, and key stakeholder interviews5. Mixed-methods Data Analysis Services: tying together the various available data sources to paint the bigger picture on key challenges for use in decision making6. Public-private Partnership Services: working across an array of community partners to foster new collaboration that drives outcomes, economic resilience, and the creation of novel campaigns, funds, and other partnership to achieve common goals

We are boutique consulting firm partnering with community based organizations, hospital systems, and public health organizations at the local, state and national level to improve health outcomes while increasing the efficiency of health care services.After 13 years supporting government and non-governmental entities use data and evidence-based methodologies to improve quality of care while cutting costs, I am excited to be launching Hager Health. A few of my recent projects include the in+care Campaign (www.incarecampaign.org), end+disparities Learning Exchange (www.enddisparitiesExchange.org), and the HIV Cross-Part CareContinuum Collaborative (H4C). All of these examples increased HIV visit retention and HIV viral suppression among its participants through virtual and face-to-face peer learning. Working in these initiatives, I develop and manage targeted learning curricula, editorial calendars, and quality improvement tools to guide participants toward better health outcomes for their patients. Our social activities are based on the idea that folks living with HIV provide a special type of support to each other by sharing their life experiences and their knowledge of local resources, benefits, and opportunities to share and give back. All are welcome! impac+NYC is an all-volunteer, social charitable organization made of, by, and for NYC’s gay poz community. All our activities are free or made accessible for community participants who have limited means. impac+NYC is party of Positive Alliance, an umbrella organization of communities like impac+NYC across the United States. I lead the other managers and serve as a formal link between impac+NYC and Positive Alliance. Nqc Campaigns ManagerNational Quality Center | Nys Department Of Health Aids Institute Oct 2011 - Sep 2017Albany, New York, UsResponsibilities I manage NQC's special group learning activities, including Quality Improvement (QI) Campaigns. The goal is to increase quality management competency and capacity to ultimately improve clinical outcomes for patients of the Ryan White HIV/AIDS Program. I have been in a leadership role throughout the development, implementation and evaluation of the Campaign process. Accomplishments NQC QI Campaigns Manager (in+care Campaign) ● Manage the largest HIV QI initiative in the United States representing 700 individual HIV providers who care for more than 35% of all HIV patients across the country. Through this Campaign, consumer retention in HIV care and HIV viral suppression rates among participants increased. ● Produce and manage the in+care Campaign curriculum and its six activities, which have resulted in: + over 55 60-90 minute unique webinars for audiences up to 150 provider, consumer, and agency staff; + analysis of over 150,000 retention in care data submissions from over 300 HIV provider organizations; + the creation of a new online HIV QI strategy sharing application where more than 150 HIV provider organizations and agencies have shared more than 900 care improvement ideas; + the formation of 25 community data collection and discussion groups across the US; and + the creation of a new national HIV consumer forum for HIV patients that has over 600 members. ● Consult with 10 regional HIV quality coaches contracted to directly advise participants on local QI projects. ● Consult on two articles for publication are in the pipeline based on the results of the Campaign to date. ● Consult on strategic planning for NQC’s next national QI campaign. The Washington EMA covers DC, Southern MD, Northern VA, and Eastern WV. Accomplishments ● Consulted with HIV providers and patients across politically and economically diverse areas to draft new regional standards of care for primary care, medical case management, mental health, and addiction services affecting USD 30 million in Ryan White Part A funding to over 25,000 HIV patients in the metropolitan Washington area. ● Assisted in agenda-setting for the CSCS committee. ● Facilitated the Delegation's contribution to the Part B Comprehensive Plan affecting USD 4.5 million in Part B funding assisting more than 8,500 people living with HIV in the District of Columbia. ● Assisted with committee work-plan development and agenda-setting. Director Of Quality ImprovementWhitman-Walker Health Nov 2008 - Sep 2011Responsibilities Directing the QI department at Whitman-Walker involved leading a small team of staff and volunteers in coaching six clinical departments and leading overall clinical and operational QI. I was responsible for leading the organizational advisory committees for QI across all stakeholder groups. Accomplishments ● Directed data management transformation at WWH from a free-clinic model to a community health center model. ● Directed credentialing for practicing privileges and provider insurance participation, which increased 3rd party payer revenue by more than 400% after 1 year. ● Negotiated reimbursement rates with insurance companies and contracted charges from lab providers. ● Consulted across all six clinical departments at WWH to design QI data collection and project reporting procedures, peer review processes, and other key activities such as: + managing advisory committees - staff QI group, consumer advisory group, board quality committee; + measuring and analyzing over 50 routine clinical and experience measures; and and consulting and leading 6 WWH QI projects. As an intern I worked alongside brilliant and innovative consultants working on the Round 8 cycle for the Global Fund to Fight AIDS; I assisted the HIV team in drafting their three subcomponents. After grant-writing was completed, I helped develop new training sets for use by the Indian staff to learn how to write in English scientific styles.
