Provide individual mental health counseling services to adults across a wide range of demographics. Utilize various treatment styles, such as CBT, Motivational Interviewing, Mindfulness, Harm Reduction, etc. Help individuals who are struggling with anxiety, depression, substance use disorder, co-morbid diagnoses, etc. Attend supervision sessions with a LMHC to discuss client progress, treatment goals, treatment plans, etc. All clinical and supervision hours will count towards the hours needed for my license as a mental health counselor (LMHC).

Job Responsibilities:-Assess and address any safety and risk concerns related to childrens’ health, safety, and well-being.-Evaluate living conditions of clients’ through home based contacts.-Provide clinically focused counseling to all family members.-Develop intervention plans.-Advocate and refer for concrete services to help stabilize the environment, improve family well-being, and reduce the future need for child welfare involvement.-Collaborate with external resources in the families’ ecology, i.e. school, treatment providers, CPS worker, probation, extended family, community members, etc.-May conduct specialized groups for children, adolescents, parents, etc.-Attend conferences at ACS offices and other community based meetings.-Complete weekly progress notes in CONNECTIONS and enter data into PROMIS.-Complete and update timely Family Assessment Service Plan Reports (FASP).-Attend weekly supervision and case conference meetings.-Carry a caseload of up to 8 families.-Attend ACS and JB trainings as required.-Meet monthly casework contact standard.-Examine families’ values, traditions, and beliefs against child welfare expectations.-Facilitate and attend Family Team Conferences.-Outreach communities and establish linkages with other groups, individuals, or organizations which may be helpful to clients.-Complete agency internal Incident Reports in Alltrac system.-Assess level of interactions of all family members in household, including other adults frequenting the home.-Make report to State Central Registry when required.-Identify and monitor high risk families on caseload who are at high risk of abuse, neglect, and further system involvement.-Advocate for cultural, racial, gender equity, and diversity; utilize families’ cultural values and strengths.-Empower families to solve their own problems.-Use empathy as an engagement tool.-Use supervision as a vehicle for guidance and support.

Provided individual counseling and case management services to adults in a SRO permanent supportive housing residence, comprised of individuals referred from shelters, funded by DHS and DMH. Maintained a caseload of 25-30 residents, the majority having substance abuse and/or mental health diagnoses. Provided clients with evidence-based counseling practices, such as: CBT and MI. Provided substance abuse (i.e. Stage-Wise Treatment Intervention and Relapse Prevention) and smoking cessation counseling. Collaboratively developed service plans with each client and periodically conducted various assessments, which addressed their individual needs. Monitored and evaluated clients’ progress for adjustments to meet changing needs. Led a weekly Integrated Dual Diagnostic Treatment (IDDT) group and a monthly Tobacco Cessation group for all residents. Assisted clients with finding appropriate levels of care for substance abuse treatment. Assisted clients with the application and intake process for substance abuse treatment programs. Maintained accurate and up to date client records (i.e. progress notes, service plans, assessments, psychosocial evaluations, etc.) in both hard chart and computerized form (utilizing AWARDS). Worked in transitional residential supportive housing CR-SRO facility funded by the NYS Department of Mental Health. The clientele were between the ages of 18 and 65 years old who had been diagnosed as Severely and Persistently Mentally Ill (SPMI). Our clients were referred to our agency either from state hospitals or prisons, where they had lived most of their lives. Our goal was to assist this population in learning how to live independently by teaching them various how to manage various aspects of their lives on their own. It is a 2-5 year program and, once the clients begin to make progress towards independence in multiple areas of their lives (such as following up with treatment providers, filling their medications on their own, and becoming independent in administering their medications without having to be under staff supervision), then we begin the process of assisting them in finding permanent, independent housing in the community.
