Hospital Social WorkerI worked with older adults who were in an acute hospital setting with complex care needs to facilitate a safe and timeous discharge. I carried out a person-centred holistic Risk Assessment to identify barriers to discharge, this includes collaborating with family members, and integrated working with inter-agency professionals including Health; Psychiatry; Psychology; OTs; Physiotherapists; Homecare and community-based resources.I recommended the appropriate intervention to enable a fast and timeous discharge. This could be via several discharge pathways, where home is the preferred option; intermediate care for a period of assessment; Long-term care where the risks have been assessed as critical and the service user is no longer able to live independently. If deemed as lacking capacity a full assessment of needs and risk will be conducted, and family supported to apply for Guardianship. This can be a long process and wherever possible if the service user is able to move under 13za of the Social Work Scotland Act, this will be applied instead.Statutory legislation includes Community Care and Health (S) Act; Social Work Scotland Act; Adults with Incapacity (S) Act; Mental Health (Care & Treatment) (S) Act and Adult Support and protection (S) Act. I am a trained Council Officer able to lead in Adult Protection investigations.

Community CareMy primary role is working with vulnerable adults, family members and other inter-agency professionals including occupational therapists, health, care managers and services. As this is a statutory role the main legislation I am duty bound by includes the Community Care and Health (Scotland) Act 2002 where the focus is on direct payments, charging for care, carers right to an assessment and joint working; Social Care (Self-directed Support) (Scotland) Act 2013; Social Work (Scotland) Act 1968; Adult Support and Protection (Scotland) 2007; PublicBodies (Joint Working) (Scotland) Act 2014 and Adults with Incapacity (Scotland) Act 2000. My role as a social worker is based upon minimal intervention, and what is in the service users best interest.I implement anti-discriminatory practice, promoting and empowering people to maintain and sustain living within the community for as long aspossible, enabling them to make informed choices as they are experts in their own lives. This may be with the aid of adaptations that Telecare may be able to provide or homecare services such as assisting with personal care. Assessments I am involved ininclude community care assessments; care reviews, and personalisation assessments. My role is to carry out an assessment of needs and risk which would ascertain whether the person met the eligibility criteria, and if so, go to the board forapproval for funds. If funds are available, my role would be to work with the service user to create a suitable package of care.I benefit from supervision on a four-weekly basis, this enhances my practice by helping me to understand and integrate theory, resulting in evidence based practice. I adhere to SSSC codes of practice at all times and I am registered with the SSSC

Children & Families Statutory PlacementMy primary role was working withvulnerable children and their families, and other inter-agency professionals including health, education, housing and other services such as addiction and families 1st. As this was a statutory role the main legislation that I was duty bound by was the Children (Scotland) Act 1995; Children Hearing (Scotland) Act 2011 and Children and Young People (Scotland) Act 2014. I employed policies such as Getting It Right for Every Child (GIRFEC), implementing tools such as My World Triangle and SHANARRI (safe, healthy, achieving, nurtured, active,respected, responsible and included) which promote and recognise the need for children to have their voices heard. I adhered to SSSC codes of practice.The experience I gained enhanced my excellent interpersonal and communication skills with people of all ages, increased my knowledge of theories i.e. Attachment theory, Ecological theory and Maslow's hierarchies of needs model. I worked with individuals, families and carers so they could make informed choices, whilst always recognising that the child's views were paramount throughout theassessment process. I shadowed social workers involved in child protection cases and attended child protection case conferences.I could plan and manage my caseload effectively, undertaking 1 kinship care assessment, 1 general assessment, and 6 social background reports. This involved home visits, interviews, taking part in multi-agency meetings and attending Scottish Children’sHearing. I gathered and recorded information in a sensitive, accurate and appropriate professional manner in line with departmental policies and procedures. I worked on duty on a rota-basisI worked under my own initiative, although this was overseen by my practice teacher. I demonstrated a non-judgemental attitude, whilst being careful that my own values did not bias the outcome of the assessment.

Primarily, my role is to support vulnerable and disadvantaged people. In my current role I support one service user who has learning difficulties and mental health problems. My role involves all personal care for the service user, administering medication, attending medical appointments and financial record keeping. Wherever possible I encourage the service user to be independent. This enables the service user to have a level of control in their life whilst permitting them to make informed choices. In doing so I adhere to the rights of the service user whilst ensuring their choices and behaviour does not cause harm to themselves or others. As part of my role I participate in supervision on a regular basis. This enables my supervisor and me to discuss policies and risk assessments which are in place and identify any potential hazards that may have arisen. If any risks are identified this may lead to the implementation of new risk assessment specific to the service user whom I support.
