Hospital Social WorkerI worked with older adults who were in an acute hospital setting with complex care needs to facilitate a safe and timeous discharge. I carried out a person-centred holistic Risk Assessment to identify barriers to discharge, this includes collaborating with family members, and integrated working with inter-agency professionals including Health; Psychiatry; Psychology; OTs; Physiotherapists; Homecare and community-based resources.I recommended the appropriate intervention to enable a fast and timeous discharge. This can be a long process and wherever possible if the service user is able to move under 13za of the Social Work Scotland Act, this will be applied instead.Statutory legislation includes Community Care and Health (S) Act; Social Work Scotland Act; Adults with Incapacity (S) Act; Mental Health (Care & Treatment) (S) Act and Adult Support and protection (S) Act. I adhered to SSSC codes of practice.The experience I gained enhanced my excellent interpersonal and communication skills with people of all ages, increased my knowledge of theories i.e. Attachment theory, Ecological theory and Maslow's hierarchies of needs model. I worked with individuals, families and carers so they could make informed choices, whilst always recognising that the child's views were paramount throughout theassessment process. I shadowed social workers involved in child protection cases and attended child protection case conferences.I could plan and manage my caseload effectively, undertaking 1 kinship care assessment, 1 general assessment, and 6 social background reports. This involved home visits, interviews, taking part in multi-agency meetings and attending Scottish Children’sHearing. I gathered and recorded information in a sensitive, accurate and appropriate professional manner in line with departmental policies and procedures. I worked on duty on a rota-basisI worked under my own initiative, although this was overseen by my practice teacher. I demonstrated a non-judgemental attitude, whilst being careful that my own values did not bias the outcome of the assessment.
